I134 - 9 & 10: Please Help

Hello,
My employer sponsored Green card for me and my spouse a couple of years ago. At that time, I had submitted an affidavit of support for my wife. So
For Question 9: Do I need to mention that I had submitted an aidavit for my wife?
Question 10: What should I mention about visa petetion?

IF at all you have provided any I134 in past for any one, including your wife, you should mentin it there.
