Q#10 and the income

hello,
1. My husband is sponsoring my mom's visitor visa for first time. he has sponsred for my green card and for my daughter.In Q#10 what he has fill.MY name and my daughters name.
2.we have a 401K acct on my husband's name and stocks.if we disclose it in the question regarding the income do we have to submit the staements of 401 and stocks.
