Attestation of Affidavit at Indian Consulate,NY

Hi,

I have to process for the new passport for my kid in India. For this purpose,
I have to send attested Affidavit by Indian Mission, USA. I am new to USA.

Can anyone please help me the check list required to submit to Indian Consulate, New Work for attestaion purpose.

I have the below documents currently in my hand:
1. My original Passport
2. My Kid's Original birth Certificate (2 original certificates-one with me and another in India)
3. I downloaded the Affidavit format from the home page of Indian consulate, but not sure whether I have to take this on a plain paper or on some kind of Stamp Paper like we do in India for Affidavit.

If anyone have idea how affidavit in USA looks like please share the same.
