DS 156- Please Answer-- URGENT

I am filling DS 156 form for my mother, and I am confused what I need to write in Spouse Name and Spouse DOB section, As my mother is widowed and in the form it says: enter the name if you are married, divorced and Separated.

Please tell me.. do i need to mention my Father name???
