DS 156- Please Answer-- URGENT

I am filling DS 156 form for my mother and she is widowed, and I am confused what I need to write in Spouse Name and Spouse DOB section, it says if you are married, divorced and Separated. Please tell me do i need to mention my Father name???

I think you need to mention your father's name.
