Ds-157

My brother (Age: 40) has an interview in chennai consulate on Nov 16. I am sponsoring him. From the doc list that he needs to carry, i see DS-157 form as a requirement since his age is btw 16 & 45. But the form which is available in INS web site says it expires on 07/30/2005 (the one in the upper right corner).

1. Is this form 157 really required for visitor visa?
2. Is it OK to fill this form which expired in July 2005?

I guess, you took appointment through TTS ?
Becuse now the new system allows to fill all the forms online and then take printout at VFS website.
Any way , yes the form u see on INS website is valid and still being used.
