Employment Verification letter

Hi,
My Employer is Company A located at NJ, but i'm working as a consultant for Client B located at PA.
Now when i request a Employment verification letter from Company A...do they have mention that i'm working at client B at PA state?

Also in the I-134 which company name and address should i fill in?

Is it okay if i fill (Q.1) residence address with a PA address, and my work address ( Q.7)with the NJ one?

Since company A hired you just use their info throughout the process.Do not complicate matters by giving the consulate extra info.
