DS 158 form requirement

Sir,

I want to know if we are required to fill up DS 158 as well along with DS 156 & 157 for applying for B1 visa. If not then what are the categories of visa that require form DS 158 to be filled.

DS158 is not required for visitor visa(B1/B2)

it is required for student visas only.
