How many I-134 forms for 2 applicants?

Hi,
Myself and my wife are applying for a visitor visa at Mumbai Consulate.
I read somewhere that in some consulates one I-134 form is required for each applicant from the sponsor.
I have already received ONE notorized I-134 form from the sponsor, with my wife's name also on it.

i just wanted to confirm if I require one more I-134 form from the sponsor in my wife's name with my name on it as well, or just one completed form will do at the Mumbai Consulate.

Any help would be much appreciated.

One form is enough. I had the same situation. One form must have both your names included there. All the best!! :)
