I just would like to add few points on arranging the list of documents. Often this fact is overlooked. This is what we did while applying for visitor visa to my parents couple of days back.
a) Bought 3 separate files in 3 different colours. One for Sponsor documents, one for documents related to mother's visa and finally the 3rd one for all documents related to my father's visa
b) Produced all documents as per the list recommended in this site.
c) Prepared a 'Table of contents' sheet listing out each document name and pasted it on the front side of each file.
d) Added sticky with simple notes such as 'Employment Letter'/Bank Statements/Pay Stubs etc....on each document. This should match the order in the table of contents mentioned above.
e) It is also a good idea to provide labels that can be seen side wise without flipping each doc.
Guess what, the consulate officials didn't even open the file! May be they just looked at the table of contents and just took the DS156/passport and photos. I don't know whether arranging the docs mattered. But it definitely made us feel more confident!
