Question in DS-156 Form

Hi

I am staying in US for past 4 years, on H1B visa. Going India for Visa stamping & vacation for 4 weeks. I have few questions in filling out ds-156 form, please advice me.

Questions. (In DS-156 form):
15. Home Address (Include apartment number, street, city, state or province, postal zone and country)
16. Home Telephone Number/Business Phone Number/Mobile/Cell Number/Fax Number/Business Fax Number

I gave my uncle's (Hyderabad) address for TTK Services to deliver the passports.
For the question 15/16, shall I mention my US address OR India (my parents address) Address OR Hyderabad Address?
