Visitors Visa Clarification reqd on filling I-134

Hi,

I have a question regarding filling the form 1-134.

The question no.2 is

That I am ---- years of age and have resided in the United States since (date) -------.

I came to the US initially on May 1999 on H1. Went back to India on 2001. Now re-entered in US in Mar 2004 again on a new H1 visa.

For the question above, do I need to specify May 1999 or Mar 2004.

Also I donot have any tax returns for the previous years.
How important is the tax return(s) while going for the visitors visa?

Any help on the above 2 questions will be greatly appreciated.

Provide the last and latest status that is "March 2004" in your case.
Tax returns are not must, however a good bank balance /financial backing helps as a sopnsor.
