Visitors Visa - Form I-134 query

I am applying visitors visa for my parents. Do I need to send separate I-134 forms for my Mom and Dad?

I think you need to send 2. It is mentioned in i-134 that 1 form is for single person.

Again, This is not a legal advise ..just what I read.

I applied for an extension of my parent's stay on Dec 31.Their I-94 is expiring on 25'th February.I just found that the airlines would not extend the tickets and without being sure of getting an extension, i didn't want to take the risk of losing the tickets.
What is the procedure for getting back the I-94 card, and how quickly , can i get it.

I used only one form for my parents and they got visa as most of the websites said one set of parents need only one I-134. But I-134 form says it is applicable to only a single person. It means you need one for dad and one for mom. So I say that to be on the safer side let them carry two separate ones.
