in my mom's DS156 i forgot to mention her last used name and surname which were both different before her marriage. Thus for the following questions i mentioned NA in her ds 156:
Other Surnames Used:
Other First and Middle Names Used:
both these were different before marriage but since we have never used them in any reference, it didnt occure to me to mention those names. so I mentioned NA. The problem is her passport clearly mentions that her name was diff before marriage and the name itself! we can't edit the forms as its not possible 5 days before date of appointment for Mumbai.
my q is, how should we deal this on the date of interview? can she say that she wasn't aware that she had to mention those names there coz she hasnt ever used those names after marriage? can someone please suggest?
Also, in the q for "who will pay for your trip", we didnt mention the name but only mentioned "SON". will that be okay too? from other answers in the form, the name of the son (me) is very clear though.
