Form I-134 Doubt

Hi i have a couple of few question

1)my parents are applying for visitor's visa. I have a question i have applied for my green card and then after that i had to renew my passport. So i changed my name in the passport from my maiden name to my surname. So now my new passport has my surname and my green card has my maiden name. All my other documents have my maiden name only. Just my passport has my surname. I am confused what name am i supposed to put in my I134.

2)I am a green card holder and my husband is a US citizen. I am working. Last week was my husband's last day at work and he is looking around so right now he is at home but he is still getting severance package till 2nd of october from his last job. My question is while filling the I-134 form under section 8 "The following persons are dependent upon me for support" do i have to put my husband's name as dependent. If no if the officer asks my parents what is my husband doing right now what are they supposed to say?

1. Fill as per your passport.
2. You have to provide the information as on the day you sign the document
( I134) , also he is not dependent on you any way.
( Definition is that person is constantly reply on you for daily living)
So the answer should be not depdent.
