Surname in DS156

Hi,
I am applying for visitor visa for my mother.
In her passport the full name is printed in the
given names section and surname is left blank.
How do I fill out the surname in the DS156 form now?
Is it ok to write NA as surname?

You can put FNU ( family name unknown) is acceptable.
