visa stamping question

I went to stamping on May 4th for h1b renewal. VO approved the visa, kept passport and told that it will take 30 days for pims verification. Meanwhile my employer withdraw the petition. So, my passport came by courier with out stamping after 30 days.
Now my question is, I am going for f1 stamping this month. What i have to write for this question in DS form.
Have you ever been refused U.S. visa??

Thanks Kabkaba. Visa was approved but my passport came by courier with out stamping. They told that not issuing visa and gave me blue slip and told the reason that petitioner withdraw the petition. Please confirm kabkaba.

If the petition was withdrawn, the employer has no job for you. There was no need to issue you a visa because you would not be permitted to enter the US.

It should be YES. Petition was approved. Visa was denied(as long as it is not stamped in the passport) .

Yes, if visa was not stamped and blue slip was issued.
