How to fill I134

I am u.s citizen taking my sister's kids (15 year old boy and 10 year old girl) to usa for summer vacation. my husband working in usa is sponsoring the trip. Do i need to fill separate I134 for each of them? how do i fill the question 3 of form

and prove their name etc as asked in Question 3 on the form,
