H1 to H4 Visa documents

Hi All,

My wife is currently working on H1 B Visa.Her job is done by Feb 15th, 2009 and she wants to transfer to H4 visa.

Can someone tell me

1) what form needs to be filled for chage of status?
2) Can we apply by ourself or better to apply with the help of attroney?
3) What supporting documents need to be sent along with application form?
4) How much time it takes for change of status (approx.)?

Since H4 application is under regular process, USCIS takes its own time for H4 processing
