Hi,
Need to know about the documents required for getting a Consent letter from Indian Embassy New York, for applying passport for my child in India.
Do we need to show the original Birth Cerificate or a Print out would suffice?
Where do I get the letter notarized? I am applying in Indian Embassy in NY.
Is it possible to do this by mail (postal service)

Please find the procedure below since I have done it 2 months back:
Fill out the Affidavit and miscellaneous form that are available in the Consulte website.
Its better you fill out the miscellaneous by typing in the machine itselt than filling it with a pen.
Affidavit, you have to fill with pen.
Get the Affidavit Notarized before you go to Consulate, for which you should have a copy of the Child's birth certificate notarized from India (for this ask your wife to get the Child Birth Certificate Notarized from India, Scan and send it . And its better to take a Color Printout of the same.)
Apply in the consulate along with your Passport (Original)
You will get the Attested form the Same day evening.
Send the Affidavit (original) to India and then your wife can apply passport for ur kid. All the best.

Notarization places will be available near consulates or I have heard that FedEx offices have notaries. It may cost from 10 to 15$ ( or may be 20$ in NY)

Be sure to have original birth certificate available when child is in the US as many services require it and will not accept a copy.
