I want more clarification on followings for my Wife and Kids (11 year old and 4 year old)

Present Occupation
Wife - Housewife
Kids - Student

Purpose of your trip
WIfe - Accompany with Spouse
Kids - Accompany with Parents

When You do intend to arrive in US
I know this is an option field, just want to conform should I fill this also?

How long you intend in USA

Who will pay for my Trip
Wife - My Husband Employer - Employer Name
Kids - My Father Employer - Employer Name

do you intend to study in is
As my kids are school going, do I need to select as Yes? In that case I need to enter school information which is not yet decided, plese confitm?

In 157 form, should I need to mention education details of my wife and kids?

I think this is correct. You could change your wife's occupation as Homemaker instead of Housewife.

If you dont have tickets yet just put a date which is a week after your visa interview date. If you have tickets in hand put the travel date. Please carry the tickets for interview If you have them.

In general, you can put yes and enter the school name under which your residence will come. But still you can enquire with people have similar experience.

I am facing 1 more problem i.e.

in my daughter passport only first name is mentioned, because in her birth certification only first name was mentioned. I recently recd her new passport. If I apply for VISA, I can not leave blank surname field, so as vfs suggested , I should enter her first name in surname and in first name I shoudl FNU. I noticed that now applicant name is displayed as FNU Disha, will this beproblem for her in USA to getting SSN and in her education (she is 11 year old).

IN case if I apply for ammendment for Passport, I need to re-apply for passport in Tatkal Scheme with an affidavit. As I have already paid for HDFC bank receipt and got the barcode no, can I use this same receipt or I need to get new HDFC receipt, as it is non-refundable amount.

Please advise. I am stuck here, not able to decide what should I do?:confused:

Usually if you apply for amendment in passport, then the authorities will not issue a new passport instead they will carry an amendment in the old passport and they will make an endrosement in it. Please check this information with passport authority officer.

Even if the last name is not mentioned the child's father and mother name will mentioned on the last page of the passport. please check it.

Further you cannot apply for SSN number for H4 applicants. This has been removed from the year 2002 onwards. For tax purpose you can get always Tax ID from IRS for H4 applicants.

I checked with the passport office, they said they can not do the ammendment, they will cancel old passport and issue new passport.

Thanks for the response. In her passport, father name and mother name is properly mentioned.

One more thing, If I continue with existing status, should we face any problem during immigration.

What will be name on her passport? If we use given name as I mentioned earlier, will it be FNU DISHA or only DISHA

I suggest you correct the following things before going for stamping since these things will have long term effects on immigration, schools etc after you arrive here.

1. It is better your kids have last name (surname) in their passport. Typically your name will be their last name. Do not worry about HDFC receipt. You may end up buying a new one. Just forego so that you do not have any problem after arriving here especially for admission into schools.

2. Regarding the schools. You should mention that your kids are going to study here. You should provide the complete address and phone number of the public school in DS 156. Your employer can enquire and find the nearest public school in their area. It is not mandatory that your kids MUST study there.

I heard that HDFC receipt is valid for one year. Why dont you postpone the visa interview date. Apply the new passport under Thakkal scheme which you get in 15 days.

It would be better if you change her name with separated first name and last name. In US it has become very particular about FN & LN. And I am not sure that name changes can be applied through Tatkal (I tried to correct my mother name in Tatkal while renewing my passport but they said I should go for normal processing. As it was days away for my travel, I kept pending and just had the passport renewed and got passport in two days. Check with the passport office). Since you will get the cancelled passport after getting the new booklet, you can use the same HDFC receipt with the old passport number. Present both the passports when you go for interview and if VO questions, you can explain the issue.
