Visitor Visa extension

My mother came to USA on 10 years multiple visa in January and we applied for extension after her six months stay. Her extension was approved but we received it now whereas she left to India in August. Actually we needed her extension for 15 days only. Is her 10 years multiple still valid thought we applied for extension because It's her first visit . Also what we should do with her new I-94. Should we send it back to INS or should we keep it with us. Will she have any problem if she comes next time. Please reply

Did she left USA ? after her I94 expiry ?
Make a copy of these document including the new I94. these will be useful when next time she travels or apply for a new visa.
Must inform the INS in details about her departure by a certified mail and keep the receipt and copy of this communication as well.

She left after 15 days after her I-94 expired. Should we send new I-94 to INS and say that she already left the country.
