expired DS-156 form

Hi !
I was looking at Chennai Consulate web site for DS-156 VISA application form. The form in that web site has expiration date of 08/30/2004. That means it is already expired. My parents have an appointment in the consulate next month. I am wondering if it is OK to use this form

Has anyone attended an appointment after August 30th used this fomr? Or is there any other link to get the latest DS-156 form? Any information is appreciated.

Dont worry, use that ..
Dept of state has not yet published new form for this and this is being used by every one.
