Refusing to issue W2

My previous employer is refusing to issue me W2 form without any given reasons.

What should I do?

You have to call IRS after 14th February, and provide your name, address (including zip code), phone number, Social Security Number, and dates of employment and the name, address (including zip code), and phone number of the employer/payer. You will then be sent Form 4852 and a copy to your employer. You need to fill that up and attach with your return.
