I am filling DS 156 form for my kid (2 years old), for question no. 39, shall I menstion Yes, and for Q 40 - My details & signature since my kid cannt sign??

40. Applicant Prepared: Shall I mention my name details for my kid?

41. Applicant Signature???? Whose signature it should have.. My Kid Thumb impression?? or my signature..
