Pls help mistake in 156 form(Murali, sirman)

Hello,

I am an Indian citizen currently residing in USA .
My mother-in-law has her appointment at Chennai consulate on september 14th for a visitor visa. We had made a mistake in 156 form on place of passport issurance as Hyderabad instead of "chennai" and AP instead of TN.

Pls help me coz somesay since the Qn1 to Qn 14 is non changible, we can strike the mistake and correct it.

Do not edit it now . Just tell VO and he will correct it himself or will ask applicant to correct it with pen.
