Question Regarding I-134

Do we need to mention the name of the person and when we filled out I-134 for the person who we have sponsored but they have not got the VISA.

i believe you do not need to mention the previous instance of I-134 if the intended visitor did not get visa. since s/he did not visit US, you do not bear any financial responsibility of his/her expenses.
