View Full Version : Surname issue in DS156 and DS157

All Senior members and others - please help

My mothers's surname in passport is (Blank)
Her given name in passport is (Last name, First name)

Please advice me as what need to be filled in DS156 for
1)Q6 :surname (as in passport)
2)Q7: First and Middle names( as in passport)
3)Q8:Other surnames used(Maiden, Religious, Professional, Aliases)
54)Q9: Other first and Middle names used

For Q8 and Q9, if there are no other surnames used and no other first and middle names used, please let me know as what generally need to filled?

5)Also in DS157, Q1: last name and Q2: first name, does it automatically by default take the names from DS156? If not can i put the actual first name and actual last name by splitting them for my mother's name, as in her passport her given name is in the reverse form (last name, first name)?

In my case, the surname is blank in passport and the given name is mentioned as XX YY. I have akways used YY as my surname in all the immigration forms and I have never had any problem. I have an F1 and 2 H1B stamping done this way. So, relax.
