Sorry i am posting my querry again as new one,as i had replied to my own thread when i wanted to add more information, hence had a feeling that readers might think my thread is already replied by someone, while its actually not. Murali/ any any other senior members/ or anyone else who has any idea please help.
I)
My mother's name :
as in the passport is as follows:
Surname: ......(Blank)
Given Names: (Full name...last name, first name) Rama Rao Leela
Filled on DS156 form as follows:
Surname: (last word of her last name) Rao
First and middle name: (her given name as in passport which is Full name in the format of (Last name, First name) Rama Rao Leela
Other surnames used: ( her first name) Leela
Other first and middle names used : (N/A)
Filled on DS157 form for my mother as follows:
Last name ( last word of her last name ) Rao
First name ( her given name in passport in the form last name, first name) Rama Rao Leela
My father's name as in passport is as follows/U]
Surname : (his surname) Kithiganur Shankar Subbaiah
Given names: ( First name, last name) krishna Iyer
[U]Filled on Ds156 form for father as follows:
Surname ( his surname as in passport) Kithiganur Shankar Subbaiah
First and Middle name (his given name as in passport in the format ( First name, last name)) Krishna Iyer
Other surnames ( his last name) Iyer
Other first and middle names used: (his first name) Krishna
I have already taken and confirmed the appointment and cannot change the info on the form. Can we change it by hand before sending it to the counsulate? what is the other best option? Is there any issue with this? I am considering to cancel and rebook the appointment for both my father and mother, if so by doing that will it allow me to change DS156 and DS157 or would the information be saved permanently no matter i cancel and rebook.
Also Murali please suggest if you see any issue with my father's DS156 form as for the names, as i see more mismatch for my mother's Ds156 and DS157. I would highly appreciate if you comment on every names i have listed. I would request any one else as well to share your ideas who has gone through this situation and attended interview.

After reading some of the forums, i got to know that if Surname is BLANK in Passport, then we need to fill FNU (First name Unknown )for First and Last Name in DS156 and fill the given name for SURNAME (section 6). Have i understood this correctly ?
My mothers given name in passport is "Rama Rao Leela"
and her surname is blank
But as i mentioned before i have filled in DS156 form as follows
Surname (section 6) : "Rao"
First and Middle names (section 7): "Rama Rao Leela"
should i had written instead as below?
Surname (section 6) : "Rama Rao Leela"
First and Middle names (section 7): FNU
Also at the end of DS156 while filling online form where in it asks as "Please reenter your Surname( as in pasport)" i have again filled as "Rao".
Did i had to fill in as "Rama Rao Leela" instead?
Please help me on this, as i feel i have done quite a few mistakes in the name sections based on what i get to know after reading others experience in their forums. Please let me know how do i correct this? Do i need to cancel and rebook a new appointment?
