View Full Version : I 134 Date field issue

I am trying to filling I 134 form from the link provided by path2usa.

There is a date field in Item (9) and (10). Since both these fields are applicable to us we need to fill them with the appropriate dates.There seems to be a software bug in the acrobat page and it puts the date in item (9) in item (10 ) and they are different. If I delete the date in (10 ) the date in item(9) gets deleted and as a result I'm unable to fill the correct dates for item(9) and item(10) :confused: . I have Adobe version 7.0.9 installed. I even uninstalled and reinstalled this. Has anyone else faced the issue I'm seeing ? Any ideas ?

Take a printout and fill it manually with a pen.

Also in DS134 i see that the cursor does not align properly for the various fields under question 7 ( for example Life insurance, cash surrender value etc). So can i take a print out and fill it manually for those fields where the cursor does not properly align to allow me to type in. Please advice. Has anyone faced this problem before, if yes please share your experience and what need to be done. I have installed acrobat reader a few times and still see the same problem only for question 7 in DS134

Yes, there is a bug in field 9 and 10 as our friend did a copy and paste between these two fields and thus they are linked even for the field values. If I get time this weekend, I will upload the fixed copy of it. Also, I will try to align the fields in question no. 7.

Thank you. Could you please let me know when you get a chance and have uploaded the fixed form. Appreciate your help. I would also check for the fixed form by downloading.

I am sorry I couldn't get time to look into this earlier. I was getting ready to fix the issues with the fillable form by downloading the latest one from USCIS website. To my delight, what I found was I-134 is now officially fillable form and all of the bug that are reported in this thread are NOT present in the USCIS version. Please download the form from this location - http://www.uscis.gov/files/form/I-134.pdf
