Why does visa get rejected

My parents visa was rejected today...no papers were asked...nothing...My sister is in USA on H1, married...My mom has been refused visa twice before...What shld we do?? My father works here...shld we reapply immmediately??

I read everything on the site, before and after refusal. My sister and her husband as told earlier, are both on H1 B. Is something wrong with their visa status. We are at sea and quite frustrated, my parents just hv to visit her. How does one convince them that they will be back? We have reapplied again and have a date after 1 month. This time we want to be fully prepared but how???
