Medical insurance - where to purchase?

Can someone guide me on where to purchase medical insurance for visitors? Is it advisable to buy from India or US? I have mixed suggestions from people because some of the insurances purchased in India work here while others don't.

If anybody has had a good experience with medical insurance purchased here, can they share their experience?
