On POE on Father's I-94 Immigration officer put mother's name. Is this a big issue ? What is the procedure to get it rectified ?

I think Name is something which the Visitor/I94 holder fill themself?
So how come it is ..?

They had name fields blank and VO filled himself. We got it corrected by visiting immigration officer at POE airport itself.
