About other Name on DS156 Form

I have question about Q.No 9 on DS156. My fathers name in passport is 'Krishnamoorthy' , so I will be entering as it is for Q.NO 7. His all other documents like House documents , business documents and bank documents is 'Krishnamurthy'.

Do you think shoud I mention' Krishnamurthy' in Q.NO 9 (other first middle Names used).

No, just provide the name as per his passport.

Now, I am confused, when you said write the same name as in passport.

If it is the same name, Isn't that I need to leave blank that field (q.no 9)

what I mean the name in the passport is ' ***moorthy ' and all other documents (bank, property, income assessments) which my father will be prouducing has ' ***murthy'

While filling up the DS-156 I will fill up the name (Q.no 7 )as it is in the passport i.e ' ***moorthy' . Question number 9 asks if there is any other name used , Since the documents my father producing has ' ***murhty' and also my passprot and birthcertificate also has my father name as '***murthy'. Should I be mentioning '***murthy ' in the Q.No 9 ( i.e other name used).
