Question on required documents

My Son has an interview on May 13 th at Chennai, I am sending all documents related to H-4, for required documents on the website it says Birth certificate of each child, so it is only my daughter DOB certificate not mine right? I have been working in US from past 3 years, but I am sending only last two years tax papers, is it must to send 2003 tax papers also? I have some problem to send 2003 tax documents.
