Do i have to fill a Application form for L2 Visa, if yes, then which form.

hi,

Order a I-765 form via phone, go to INS website and see customer service number. When u order, order a signature card along with the form, which is very important. The signature card usually comes with the form but sometimes it gets left out and submit the form to the service centre specified.
