Help me regarding Family visit of USA

Hello Friends,

I, Mr. Naresh Bhalala (Indian Citizen, Age:48 Years). This is regarding a USA Visit with my family.

I would like to apply for the USA Visitor VIsa (B2) for:
1. Myself (Age: 48)
2. My Wife (Age: 42)
3. My Son (Age: 21)

I'm sure about my family details to be mentioned in Affidavit of Support (I-134). but, not sure about Visa application Forms (DS 156 and Ds 157) and Fees.
1. What about VISA Application Form -----> I should fill only a single Visa application Form (DS 156 and Ds 157) OR
I need to fill individual for each of three?

2. What about Fees ----> For total 3 above family members, I only need to pay single application fees only ? (Visa Application Fees Rs. 4800 + Admin Fees Rs. 322)
Or it should be multiply by 3 ?

3. Please correct me, for INDIAN CITIZEN, Visa Issuence fees payment is cancelled.

You need to fill three seperate Application forms (Both - DS156 & DS157) and get three hdfc recipts for each applicant so you have to pay fees for each of the applicant ,Also Visa Issuance fees is cancelled now
