where can i download I134 form?

Thanks. finally i found out. Is it o.k to print that form. i mean i took the prinout of the form and i filled with black Ink as mentioned in the top of the form . is it o.k?

You can actaully fill it online and then take the print out. It is a fillable form.

when i try to erase those sample data and try to fill my data. it says it's not fillable. please clarify my doubt.
