Property affidavit format for parents

Hi,

We are planning to prepare the property affidavit for both mother and father. Though house is in name of father, my mother used to work and she is receiving pension and has bank accounts/LIC/NSC etc in her name also.

So in the same affidavit we had like to include both mother father details in the following format

AFFIDAVIT OF PERSONAL PROPERTY, INCOME AND ASSETS

Name and address of Applicant
Father's Name and Mother's name

This sounds very good. We prepared a single affidavit for both my father and mother though some of the properties on my father's and some are on my mother's name.
