View Full Version : Incorrect Passport Issue Date and Expiry date in DS-156

Hi Readers,
I filled incorrect passport issue & expiry dates on my mom's DS-156 application.

I live in US and was filling the application as she was telling me over the phone and there was a miscommunication about the dd/mm stuff. She said it as dd/mm and I assumed it was mm/dd and finally I entered the incorrect dates. (I guess I was too excited about the regional appoinments being available, that I overlooked and didn't double check). The appointment date is confirmed now and so the fields 1 thru 14 in DS-156 are now uneditable.

Do I have to fill another application now? I did send an email to the consulate, but haven't received any reply as of now.

Has anyone been in similar situation? I just want to know if it affects my mom's visa chances adversely. I don't want a mad VO yelling at her for filling such a simple form incorrectly.

Any replies are appreciated.

Why don't you try to cancel the current appt and fill a brand new application? Since there are so many dates available, it will not be a risky move to cancel the existing appt. Your call.. good luck!!

I don't think the mm/dd vs dd/mm will be an issue. Your mom can inform this to the VO at the time of interview. But, since the appt dates are available, and that you have realized early on, it may be worth while to fix the error while more appts are available rather than taking a chance (if you had realized just few days before the appt then I would have suggested not to cancel the current appt)

Thanks for your reply Jaya. I'm actually trying to schedule her appointment in Nov itself. There is just one date left in Nov as of now, and I'm not sure if it will still last if I go back to cancel the appoinment and reschedule it (this would mean entering all those forms all over again.)

I just want to know if I can have her fill another DS-156 form (available at https://evisaforms.state.gov/ds156.asp) at the time of the appointment. Anyone done that and were successful with the visa?

I don't have real-time experience with this. DS156 is something that consulates save for future use. So, they keep the soft copies ONLY. A hand filled form from govt website may be okay, but your parents have to inform this to the visa officer so that he will correct the dates on the form in his computer.
