DS-156....can anyone pls clarify?

i was reading the forum and read the discussion on "where to print the DS-156 from?" i cheked the http://evisaforms.state.gov/ but cant find any 3 page DS-156 form. i can only see a 2 page form there as we have on the VFS website. can anyone clarify this pls?

shd i jus go on with the vfs forms or shd i make a new form. if yes can anyone let me know where to find the 3 page form because i checked and could nto find it on the evisa website.

i think you'll find the 3rd page when you actually download the DS-156. download the form and check.

Also 3rd page will have information about your additional visa'a and additional visits from Q29, 30 and 31. If there are no additional information in these questions, you may not have 3rd page at all.
