Hello,

If we want to advance our appointment, after cancelling existing appt, do we need to fill out the forms all over again? Given the amount of time we would need to fill out the forms all over again, have people been able to succesfully get an earlier date? Isnt there a bigger risk of the appointment getting pushed further away? Experienced folks, please advice.

if you have more than 1 date/days available them go a head and cancel the appointment. Otherwise you are taking a chance.

If you are rescheduling - which can be done only once, you can resuse the same form. Otherwise you need to fill the form again. Do not cancel if you see only one date.
