I researched sooo much of this forum and found many answers but
still have some doubts.
Premium processing is on for me (H1B), so pls treat this as urgent.

DS156:

14: National Identification no? ...Can this be my PAN no.?

21: Present Occupation?... This means my present Position in my
current company or the title that im getting frm the US company?

24: At what Address will you stay in the US?...I have no idea
about this?

25: Name and Tel. nos of person in US Who you will be staying
with or visiting for tourism or Business?....Do I have to fill
this and if yes who should be mentioned here?

29: Have you ever been to U.S?
When?_______ For How long? ______
...I have been to US twice on B1. Should i mention two periods
in the "When?___" and respective duration of stays in the "For
How long?____"?Pls clarify.


DS157:

9: List all countries You have entered in the last 10 years?
...Should i mention those countries where i Landed for break
journey(on the Airports only) when I had my last two Business
trips to US?

13: List all professional, Social, Charitable Organizations to
which you belong (Belonged) or Contribute (Contributed) or with
which you Work (Have worked)... Should this have the name of the
Company I am working with and my previous companies?

17: List all education Institutions you attend or have attended.
Include vocational institutions but Not elementary schools...
This should start from 10th Grade.Right or wrong?
I am waiting like a 5 year old for your replies.

21. Write the present occupation - WORKING
24. Ask this to your US employer.Generally employer's guest house where you would stay
25. Mention your employer's name and contact
29. YES

9: No Just mention US
13: You can mention your current employer here(Not sure about this.I think you can write N/A here.Please confirm this with someone else too)
17: Start with your latest degree and go backwards for e.g B.E, 12th, 10th etc

14. N/A...but I thought that PAN no. is a National Identification no.?

25. Mention your employer's name and contact...but I am neither going on Tourism or Business, I am going to Work on H1B. So shall i fill it or not? Pls tell me.

29. YES...So in "When___?" Shall i mention 2 different periods like 1st Jan to 30th March & 1 June to 31st August? And in "For How long?____"? I should mention like 88 days and 90 days respectively? I am so confused abt this one.

14.Well a passport can also be a national identification number right?

My feeling says write " N/A".I wrote N/A in my case

25.Ask your employer for his guest house address and write someone's name who is staying there or write employer's name or some HR person's name and contact
