I am trying to take an appointment for visitor visa for my father.
The name (as in passport) for my father is Mohan Lal. There is no surname. I mean surname is there, however, there is no surname in passport.
what should I fill in question #6 "Surnames" in DS156 form? It says that the field is mandatory. Can I write NA or none?
Appreciate a quick response on this.

Also u can do an observation done at the passport office to correct this. Since u are not changing the names, its possible. Its a one day process with minimal fee.

The screen just before the DS-156 screen clearly states that if there is no surname in the passport, then everything in the Given Name(s) listed in the passport should be filled in the surname field, and "FNU" should be filled in Q #7

Its true, But FNU is sometime problamatic later. So its better to get the observation done if you have time to do this.

If you dont have enough time for corrcting, put every thing as surname.
I dont see any major problem for parents, besuce of the name in visa stamp.
It is recommended to have proper and correct information though.
( the problem can be for people who comes on work visa and difficul to get SSN/ driving lic etc because visa stamp has different name etc)

i have further queries on this:
1. what does FNU mean?
2. I checked my father's passport, the name (as in passport) is:
Given Name: MOHAN LAL
Surname: blank
3. Please note that I have already paid the fee at HDFC bank by submitting the xerox of first page of my father's passport and filling that form. Is it advisable to get the suname added now in passport? If I do that, can the same fee and HDFC bank receipt be used to take appointment?
Please advice the best course of action. I need advice for filling question #6 and #7 in DS156 form.

u wld need to provide name info exactly as in passport - the visa is also issued in the same name as in the pp
